Salary £23,465 – £24,679 | £12.00 – £12.62 per hour

Due to growth, we are looking for motivated people to join our Supported Living Services. You will work within a team to support a young lady who has learning disabilities to live in her own home and the community. You will play an important part in making sure she lives life to the full.
By providing personal care and support, you will promote maximum independence and social skills, whilst exploring positive opportunities for her to access community life.

Some of your main duties will include:
- Supporting a young lady to access agreed programmes as directed by their personal plans
- Be part of a team to ensure her personal plans are delivered, monitored and evaluated
- Enabling opportunities to develop activities in the wider community
- Assisting her with personal care needs and contribute to regular health care assessments
- Promoting positive practice of Safeguarding, Health & Safety and Mental Capacity within the workplace
- Maintaining accurate and up to date records as required and assist in the monitoring and evaluating of the service provision and plans
