Job Description and Person Specification

You will work to deliver services ‘hands on’ as the leader of the team, helping this young lady to lead a fulfilling life within her home and community. You will work to shape the service and to ensure high quality outcomes are achieved across Supported Living Services as a part of the wider management team.
You will deliver services on rota and the management task is to lead the team in delivering excellent support to the young lady. You will be responsible for ensuring that agreed outcomes from individual person-centred plans are met and to maintain excellent working relationships with the family, key professionals and the wider community.

Some of your main duties will include:
- Taking a lead role in co-ordinating day to day tasks and allocation of activities
- Ensuring finance, medication and compliance checks are undertaken as required
- Promoting pride, individuality and a welcoming atmosphere within the individual’s home
- Contributing to the maintenance of their health and wellbeing through the delivery of appropriate programmes of care and support
- Maintaining and promoting a positive environment by developing professional working relationships
- Working within the principles of Rights, Independence, Choice and Inclusion
- Supporting the individual to access agreed programmes as directed by their personal plan
- Ensuring accessible recording of activities and personal development
- Assisting with personal care needs and contributing to regular health care assessments
- Working closely with family, carers and other professionals to meet the needs of individuals
- Maintaining accurate and up to date records and assisting in the monitoring and evaluating of the service provision and plans
